This undersigned in applying for a playing membership acknowledges and agrees to the following:
1.
The undersigned has received a copy of the By-Laws of Benton Golf &
Country Club, Inc. amended on

The current dues schedule applies to every playing membership as from
time to time amended;
a. Playing membership-Three Hundred Fifty
Dollars ($350.00)
b.
Current dues-Two Hundred Forty Dollars ($240.00) per quarter, to be
prorated for the first quarter the member joins and paid on the 1st
day of each quarter thereafter. Any dues
payment that is not paid by the 15th day of each quarter shall bear
a Sixteen and 50/100 Dollars ($16.50) late charge on the 15th day of
each month. Quarterly bills are mailed
in January, April, July, and October.
Failure to receive a due statement shall not be a defense for
nonpayment.
c. Members may rent the clubhouse for a rental fee of One
Hundred Twenty Five Dollars ($125.00) which includes a damage/clean-up deposit
of Fifty Dollars ($50.00). The member renting the clubhouse shall be
responsible for basic clean-up and any damage to the clubhouse or golf course
by the member or their guest during the rental period. If the clubhouse is basically cleaned up and
there is no damage, the deposit will be returned to the club member.
d. The swimming pool may be rented during
times other than regular operation for a fee of Fifty Dollars ($50.00) up to
two (2) hours. A certified lifeguard secured
by the member is required to be present during any time the swimming pool is
rented. Any rental for greater than two
(2) hours shall require an additional fee of Twenty-Five Dollars ($25.00) per
hour or any part of an hour.

Any player member may terminate their membership by notifying the
Treasurer in writing prior to the first day of the month when the membership is
to be terminated. All dues must be paid
current through the end of the month in which the membership is terminated in
order for the member to withdraw in good standing. The membership may be reinstated for a period
of ninety (90) days after its terminated if all dues
for the prior period are brought current and all playing memberships have not
been sold. After ninety (90) days, a
withdrawing member may apply for a playing membership, but must pay a new
membership fee and any delinquent amounts that were due upon withdrawal of the
application.

Any playing member that fails to make any monthly payment fifteen (15)
days from its due date, shall have the playing
members’ name posted in the clubhouse and the playing member and anyone playing
on that membership will not be allowed to use the club facilities until the
delinquent amount and late charges are brought current.
5.
The playing member acknowledges that the operation of a golf cart by
anyone less than fourteen (14) years of age creates a danger of property damage
and personal injury and agrees not to allow the operation of a golf cart by any
child under fourteen (14) years of age in the absence of a member twenty-one
(21) years of age or older. Any child
fourteen (14) years of age or older that has not received a driver’s license
must have filed with the club a golf cart permission form or that child will
not be allowed to operate a golf cart on the premises. The undersigned further agrees to indemnify
and hold harmless Benton Golf & Country Club, Inc., its board of directors
and officers from any claim as a result of death, property damage or bodily
injury resulting from the operation of a golf cart by anyone that is not a
licensed driver, which indemnity shall include court costs and costs of defense
incurred by the club, its directors or officers.

The member acknowledges that this is only a summary of portions of the
By-Laws and dues schedule and that the member and anyone playing on their
membership are fully bound by the terms and conditions of the By-Laws and fees
schedule as from time to time amended set out herein.
